Who is my Claims Company?
HealthSmart Benefit Solutions formerly known as Klais & Company, Inc. is responsible for processing your claims. Their dedicated claims analysts and customer service representatives can address any questions you have on an outstanding claim or on how a claim has been paid.
How Do I Submit a Claim or Request Reimbursement?
When outside of the United States, you will likely be asked to pay for your medical care first and then will need to seek reimbursement. When you submit claims for reimbursement, you will need to have the itemized bill(s) translated into English and the "International Studies Claim Form".

UnitedHealthcare StudentResources will be responsible for paying claims. Their dedicated claims analysts and customer service representatives can address any questions you have on an outstanding claim or on how a claim has been paid.
Online Access to Account Information
UnitedHealthcare StudentResources insureds have online access to claims status, EOBs, correspondence and coverage information via My Account at www.uhcsr.com. Simply select the "Create an Account" link from the home page at www.uhcsr.com. Follow the simple, onscreen directions to establish an online account in minutes. Note that you will need your 7-digit insurance ID number to create an online account.

A claim form is not required. However, you can download one by clicking on the following link:
Medical Claim Form
To request reimbursement for a medical claim, mail the original invoice, proof of payment, copy of insurance ID card or Acceptance Confirmation to Student Resources. Please retain a copy of all mailed documentation for your records. Approximately 30 business days are required to process a medical claim.
