ENROLL YOURSELF AND ANY DEPENDENTS IN THE VISION PLAN

Funded PhD Students: You will be automatically enrolled in both the Delta Dental and EyeMed Vision plans at no cost to you. You can add your dependents to your EyeMed Vision Plan at no additional cost to you, however there is an additional cost to add dependents to Delta Dental. If you want to enroll your dependents in these plans, please complete the applicable online enrollment form by the February 28, 2023 deadline.
